I authorize Simba Group's approved technicians to come into my residence to perform the requested service, under the supervision of my designated representative if I am unavailable.

I further agree that any concerns relating to loss, theft, or property damage must be reported immediately in the presence of Simba Group's representatives during the service visit. Simba Group shall not be held liable for claims that are not reported at the time of service.
